Usage
Base

Note: all fields must be present in order for the userbox to display properly, and the first and last fields must be filled out.
This userbox may be set to name a villager and your birthday. Replace <Villager> in the above code with the name of a villager that shares your birthday, and <Date> with your birthday. <Species> and <Personality> may be left empty if desired.
Customization
The species and personality of the villager may also be added to the userbox:

Replace <Species> with the villager's species, and <Personality> with their personality. This information can be found on their wiki page.
